Service Authorization

Service Authorization I, the undersigned, authorize [Business Name] to perform the following work: [describe the service]. Estimated cost: $[amount]. I understand that additional work found necessary during the job will not proceed without my approval, and that the final invoice may vary from the estimate for approved changes only. Payment is due on completion unless agreed otherwise in writing. Materials removed during the work may be discarded unless I request their return today.
