Enrollment Terms & Conditions 1. ENROLLMENT. This enrolls [student name] in [class name] at [Business Name], [start date] to [end date], [schedule]. 2. TUITION. $[amount] per [term/month], due [when]. A registration fee of $[amount] and materials fee of $[amount] are non-refundable. 3. WITHDRAWAL & REFUNDS. Withdraw before the [2nd] class for a full tuition refund; before the [4th] class for 50%; no refunds after that. Missed classes are not refunded; [1] make-up class per term is offered where space allows. 4. CONDUCT. Students follow class rules; repeated disruption after a parent conference may end enrollment with a prorated refund. 5. MEDIA. Photos of class activities may be used in our marketing ONLY if the photo-release box is checked: [ ] I consent. 6. MINORS. Parents sign for students under 18, keep emergency contacts current, and pick up within [15] minutes of class end (late-pickup fee $[amount] per [15] minutes).
